This is a long form and we know that parents are very busy people but we assure you that all of the information we are asking for is either a government requirement or helpful to us in making your child's entry to Reception as smooth and enjoyable as possible. 

You can tackle this in chunks and save your progress as you go, or settle down with a cup of tea and just plough straight through! We do appreciate you taking the time to complete this fully.   

Treetops bookings for new Apple Class children will open on Tuesday 1st July 2025

Some of the information requested on the Treetops registration form is also included in the main school registration form however we would ask that you complete this form fully as the data will be uploaded onto a different system.
